Is Penile Filler Safe? What a Dallas Urologist Wants You to Know

If you have been following the news out of Dallas this fall, you may be wondering whether penile filler is safe at all. It is a fair question. A lawsuit filed in Dallas County alleges that a 36-year-old man died of a pulmonary embolism after his fourth round of penile filler injections at a local wellness center. The defendants deny the allegations, and the case has not been decided.

Here is the short answer from our side of the exam table. Hyaluronic acid filler, injected by a trained physician who knows penile anatomy and is prepared for a complication, usually causes mild, short-lived side effects. Serious complications are rare. Rare is not never, though, and the serious ones tend to trace back to the same few things: who is holding the syringe, what is in it, and what happens in the hours and days afterward.

When the Dallas Observer asked Dr. Michael Wierschem, our founder and a board-certified urologist, about it, his advice was simple: “I’d be really careful with who you go to.” This guide explains why, and what to ask before anyone treats you.

What Penile Filler Is, and What It Is Not

Penile girth filler is a gel injected under the skin of the shaft to add circumference. Many clinics use hyaluronic acid (HA), the same family of filler used in lips and cheeks. HA is temporary. Your body breaks it down gradually, and an enzyme called hyaluronidase can dissolve HA filler in some circumstances if treatment is needed.

Two facts are worth knowing before you go further:

  • No filler is FDA-approved specifically for the penis. HA fillers are approved for specific areas such as the face and lips. Using them for girth enhancement is off-label and requires an informed discussion of risks and alternatives.
  • Permanent fillers are a different category. Silicone, paraffin and other permanent materials can cause hard lumps, deformity and infection that may require surgery. The Sexual Medicine Society of North America discourages permanent fillers for cosmetic penile enhancement.

Penile Filler Risks: What the Research Shows

Penile filler is still an emerging field. In its 2024 position statement, the Sexual Medicine Society of North America said temporary fillers such as HA may increase girth with a moderate safety profile, while calling for larger and longer studies. That is an encouraging but not definitive evidence base.

Common, short-lived side effects

  • Swelling and bruising for the first several days
  • Tenderness at injection sites
  • Small lumps or uneven areas that may require assessment or adjustment

Less common complications

  • Infection
  • Nodules requiring treatment
  • Filler shifting or settling unevenly
  • Vascular compromise, when blood flow is obstructed, potentially damaging tissue

Why the Person Injecting You Matters More Than the Filler

The Dallas lawsuit alleges that the injector was not licensed to practice medicine in Texas and that the patient was not examined in person after reporting a lump, swelling and numbness. These remain allegations, not court findings. Dr. Wierschem told the Dallas Observer that he sees men seeking help after treatment at other clinics.

Here is what an appropriately trained physician should bring to the procedure:

  • Anatomy: An understanding of the blood vessels, nerves and tissue planes involved.
  • Product judgment: Choosing an appropriate product, amount and placement, including knowing when not to treat.
  • A complication plan: Immediate assessment, appropriate supplies and a clear escalation protocol if blood flow is affected.
  • Real follow-up: A way to arrange prompt in-person evaluation if pain, discoloration, numbness or swelling develops.

At Dallas Men’s Health, Dr. Michael Wierschem performs girth treatments himself. He has extensive experience in urology and penile enhancement procedures.

8 Questions to Ask Before Any Penile Filler Treatment

Ask these questions during your consultation. A qualified clinic should be willing to answer them clearly.

  1. Who will perform the injections, and what is their license? Verify a Texas physician through the Texas Medical Board.
  2. Is a physician on site during treatment? Ask who will respond immediately to a complication.
  3. What exact product will you use? Ask for the brand name and documentation.
  4. Is it a temporary hyaluronic acid filler, and can it be dissolved? Ask what reversal can and cannot accomplish.
  5. Do you have hyaluronidase and a vascular-complication protocol on site? Request a specific explanation.
  6. Will you examine me in person before treatment and if symptoms develop?
  7. How many procedures have you performed, and can I see authentic results? Review the before and after gallery.
  8. Will you tell me if I am not a good candidate? A sound consultation includes the option not to proceed.

Red Flags That Should End the Conversation

  • No named clinician or unclear credentials
  • Booking without a medical history or examination
  • Refusal to disclose the product
  • Permanent filler or injections outside an appropriate medical setting
  • Pressure to purchase more syringes
  • Guaranteed measurements or outcomes
  • No clear after-hours contact or complication process

Warning Signs After Penile Filler

Some swelling and bruising can occur during early recovery. The following symptoms require prompt attention:

  • Severe or worsening pain
  • Skin turning pale, blue, gray or dark
  • Spreading numbness
  • A growing lump, especially with warmth, redness or tenderness
  • Fever
  • Chest pain or shortness of breath

Urgent safety warning: Seek urgent in-person medical evaluation for worsening pain, skin color changes, spreading numbness or signs of infection. For chest pain, trouble breathing or other life-threatening symptoms, call 911 immediately. Do not wait for a routine callback.

Already Had Filler Somewhere Else?

Men sometimes seek second opinions after treatment elsewhere because of lumps, uneven areas or unanswered questions. Dr. Wierschem can evaluate the concern, review available records and discuss options for correcting filler done at another clinic. Bring the product name and treatment records if available. You do not need to return to the original provider for a second opinion.

Frequently Asked Questions

Is penile filler safe?

When a trained physician uses a temporary hyaluronic acid filler and is prepared to manage complications, most reported side effects are mild and short-lived. Serious complications are possible, so provider qualifications and follow-up matter.

Is penile filler FDA-approved?

No filler is FDA-approved specifically for penile enhancement. HA fillers have approvals for certain other anatomical areas, and penile use is off-label.

How long does penile filler last?

HA filler is temporary and gradually breaks down. Duration varies by product, placement and individual factors, and maintenance may be needed.

Can penile filler be reversed?

Hyaluronidase can dissolve HA filler, although reversal may not resolve every complication. Permanent materials such as silicone cannot be dissolved this way.

Can penile filler cause erectile dysfunction?

Erectile dysfunction is not an expected routine outcome, but complications affecting tissue, nerves or blood flow could affect sexual function. New erection changes after treatment warrant medical evaluation.

Who should perform penile filler in Texas?

Choose an appropriately licensed, experienced medical professional with relevant anatomical training, immediate complication-management capabilities and a clear follow-up plan. Verify physician credentials through the Texas Medical Board.

What should I do if something feels wrong after filler?

Seek same-day medical assessment for worsening pain, skin discoloration or spreading numbness. Call 911 for chest pain or difficulty breathing.

Can I get a second opinion on filler done elsewhere?

Yes. A urologist can assess symptoms, review treatment records and discuss possible next steps.

Key Takeaways

  • Temporary HA filler can increase girth, but every injection procedure has risks.
  • No filler is FDA-approved specifically for penile enhancement.
  • Permanent filler materials pose additional long-term concerns.
  • Ask about the injector’s credentials, the product and emergency procedures.
  • Worsening pain, discoloration or spreading numbness warrants urgent evaluation.
